Local Initiatives
Unite New York is a coordinated care network of health and social care providers. Partners in the network are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite New York is sponsored by a variety of funding partners currently led by Adirondack Health Institute, Alliance for Better Health, AIRnyc, Central New York Care Collaborative, Hillside-Catholic Charities, and Public Health Solutions. Under the Unite New York umbrella, local regional networks include Community Referral Network, Unite NYC, The 360 Collaborative, and the CNYCC Referral Network.
Healthy Together and ADK Wellness Connections
The Healthy Together network, sponsored by Alliance for Better Health, launched in April 2018 and covers a 6-county region (Albany, Fulton, Montgomery, Rensselaer, Saratoga, and Schenectady counties). The ADK Wellness Connections network, sponsored by Adirondack Health Institute, launched in October 2018 and covers a 9-county region (Clinton, Essex, Franklin, Fulton, Hamilton, Saratoga, St. Lawrence, Warren, and Washington counties). The two networks overlap in Fulton and Saratoga, for a total of a 13-county region. Since launching, over 300 partner sites have been added to the networks across the 13-county region, with more than 4,500 clients added and 5,100+ needs referred. There are three coordination centers across the network which receive assistance requests from individuals in need.
The 360 Collaborative
The 360 Collaborative network, sponsored by Hillside-Catholic Charities Behavioral Health Care Collaborative, launched in April 2018. The
collaborative has grown through mergers with Your Health Partners of the Finger Lakes-Southern Tier Behavioral Health Community Collaborative, New York Serves, HHUNY (Health Homes of Upstate NY), and the RMAPI Adult Mentoring program. In 2019, NYServes-Upstate, a network for veterans and the military-connected community, joined the network. Between stakeholders, service is provided across an 11-county region (Monroe, Wayne, Livingston, Ontario, Seneca, Yates, Steuben, Schuyler, Chemung, Tioga, and Broome counties).
Unite NYC
Unite NYC is a coordinated care network of health and social care providers that is launching on April 14th and sponsored by funding partners currently led by Public Health Solutions and AIRnyc. In 2018, our work with Public Health Solutions began as a small food security grant-based pilot (FNS Bundle) in the Bronx. The current city-wide expansion is part of a rapid response to COVID-19.
CNYCC Referral Network
